You need to fill in this form if you are going South and working in the field.

• Please submit the completed form before the deadline date specified by Operations
• Copies will be distributed to you, your GA and the Operations Unit
• Additional copies, printed and in other formats, can be requested via this form
• All printed maps will be laminated
• You will receive your copy(s) at Rothera (you can let us know via this form if another collection location is more suitable)

If your request is for more than one map for the same project, you only need to complete the details section once. You will be asked if you wish to add additional maps at the end of the submission process.
